We only ship to the continental USA. There is a $50 minimum order. All orders are required to be placed on our web site prior to shipping or pick up, or surcharges will apply.
- Fill out page 1 of the Uniform Sales and Use Tax Exemption document. Please make sure it is fully completed and legible.
- For your personal records & reference, here is a link to the complete Uniform Sales and Use Tax Exemption document (updated as of 01/22/2018)
- *Email or fax us a copy of your State Reseller’s License or Certificate. This is issued from your State or County Tax Authority.
- *Email or fax us a copy of the front page or cover of Certificate of Business Insurance including policy number
- Listing of name of principles of business organization, their titles, all business addresses, FEIN.
- Register an online account on our wholesale web site.
Upon satisfactory submission, review, and approval (3 business day minimum), we will contact you of the approval.
